5-year-old male with a 2-year history of intermittent abdominal pain after eating and painful defecation. Stools are every 4–5 days, hard, and pellet-like. For the past 3 weeks, he has had episodes of involuntary soiling with watery stool despite being potty trained. Diet is poor, limited to processed/fried foods and excessive milk intake 4–5 cups a day, with refusal of vegetables and water. Stressor includes recent parental divorce. No past medical history or medications.

M.K, 4-year-old male is presenting with his father for concerns of abdominal pain on and off the last 2 years with changes in his bowel movements. Father reports pain starts after eating and goes away after a few minutes. He has not noticed a change in his appetite or behaviors. The child diet consists of fried foods, no vegetables, 3-4 cups of milk a day. His bowel pattern is 4-5 days with small, pebble stools and over the last 3 weeks he has been leaking stool in his underwear. He denies any nausea, vomiting, diarrhea, foul-smelling or blood in the stool. The child has no medical problems, no surgical history or allergies. He has not attempted any treatment for abdominal pain or bowel leakage.
