J.M. is a 23 y.o. female with a PMH pertinent for UTIs who presents with c/o dysuria x3 days with associated frequency, urgency, and “yellow-creamy” vaginal discharge. She denies fever/chills, rash, and genital lesions. Endorses unprotected sexual contact with multiple partners. Exam findings reveal she is afebrile with no suprapubic or CVA tenderness. Her cervical os is noted to be friable with mucopurulent vaginal discharge.

Pt denies fevers, lesions/ sores, itching, and hematuria. Pt reports having unprotected intercourse with multiple partners. PE reveals cervical friability and mucopurulent discharge. Hx is significant for bladder/yeast infections.

Associated symptoms include urinary urgency, and yellow, creamy, non-odorous vaginal discharge. She reports more than one sexual partners, denies STI protection. Mucopurulent discharge from the cervical os, and cervix friable were noted on the physical exam.”
