Problem Statement

E.T is a 65 yo old obese male with a chief complaint of fatigue/tiredness and nocturia. Reports not getting enough sleep due to urinating 3-4 times a night and 6-7 times during the day. In addition, reports intermittent blurry vision and unintentional 8lb weight gain, polydipsia and polyphagia. Denies history of smoking, dizziness, nausea/vomiting. Reports stopped drinking beer on the weekends x1 month ago to help with urinary frequency. Past medical history includes benign prostate hypertrophy. Physical exam elevated blood pressure, glucosuria, mild proteinuria, nonfasting glucose 230mg/dL, diminished sensation light touch to bilateral ankles, and monofilament diminished to bilateral ankles.
