Description
J.R. is a 79 year old African American female, presenting today with her daughter, who voices concerns of increased and progressive confusion and memory loss in her mother. The patient is minimally cooperative during the interview. Her history is significant for HTN, HLD, and CAD with stents. Her daughter states she has had increased difficulty with impaired judgment and organization such as leaving the car on while she goes in the store, and forgetting to pay her bills. Her symptoms are accompanied by behavioral and personality changes as well as impaired ability to preform ADL’s such as maintaining hygiene and appropriate nutritional intake.
