HH is a 57-year-old male with the chief complaint of high blood pressure. Reports tobacco smoking, weight gain, and poor eating habits. Denies headache, palpitations, and shortness of breath. Pertinent positive for hypertension, AV nicking, and overweight. Pertinent negative for clear lung sounds in all lobes, normal heart rate and rhythm, and soft, symmetric, non-tender abdomen.

Problem Statement for Attempt

Harvey Hoya is a 57 year old hispanic male, works in construction installing flooring and cabinets presents today for follow up on elevated blood pressure of 170/92 at recent community health fair. He is a current 1 pack per day since age of 20 smoker. He is overweight with a BMI of 28.8. He takes OTC ibuprofen for knee pain. He denies SOB, fatigue, headache, chest pain, or edema. His physical exam is notable for BP of 178/98 right arm, 172/94 left arm with a normal pulse pressure. AV nicking was also noted. He has no PCP and does not have any prescribed medications. He has significant FH of hypertension.
