Felipe Garcia, a 57-year-old Hispanic male presents to the clinic due to high blood pressure (170/92) encountered during community Health Fair. Patient has insufficient preventative health with no established history of primary care provider. Patient is overweight BMI-28.8 and works 5 days a week. Consumes high added salt diet and caffeine. Present with family history of Hypertension, Heart Attack and DM. Personal history of smoking a pack/day for 35 years and occasional drinking alcohol. Denies drug abuse and tobacco use. No known allergies. History of Rt. knee surgery. Patient has occasional epigastric pain and Rt. Knee pain. Patient is on Ibuprofen PRN for knee pain and Omeprazole PRN for epigastric pain. Patient denies headache, palpitations, blurred vision and nausea.

F.G. is a 57 year old overweight male presenting for an evaluation of high blood pressure. Patient reports “stomach burning” and “achy knees”. Reports a poor diet, smoking a pack of cigarettes per day for 35 years and a family history of hypertension and diabetes. Patient denies chest pain, palpitations, shortness of breath, headaches, vision changes, dizziness or numbness or tingling in extremities. Upon examination pertinent positive findings include blood pressure elevated with left 172/94, right 178/98 and arteriovenous nicking noted. Pertinent negative findings include clear lung sounds in all lobes, no swelling, no facial droop. Patient was notified of elevated blood pressure in past and recommendation to improved diet, but did not seek additional treatment.

FG is a 57-year-old male with a new elevated blood pressure. The physical exam is notable for a BP of 170/92. A smoker of 35 years. He has a family history of HTN and CAD. He is overweight at 195 lbs with a BMI of 29.5.
Arteriovenous nicking is present. Past medical history of gastritis and use of NSAIDS. There are some risk factors that include a poor diet, alcohol use, sedentary lifestyle and history of smoking.
