AL is a 57-year-old male who presents with high blood pressure for an unknown amount of time with no prior medical care. Patient states he is a 35-pack year smoker and has a poor diet. Patient denies chest pain, shortness of breath, dizziness, headache, and lightheadedness but did complain of some chronic knee pain for the past few years. Patient presents hypertensive at 172/94, with AV nicking, and overweight. Heart sounds are normal, lung sounds normal, radial pulses present and equal bilaterally, knee exam normal. Patient has a significant family history of diabetes, stroke, and hypertension.

He has a one-pack-per-day smoking habit and presents for evaluation of elevated blood pressure. The patient reports that his blood pressure was measured at a recent health fair, where a nurse informed him that the reading was elevated and advised him to seek medical follow-up. He recalls being previously encouraged to make dietary changes to help manage his blood pressure but admits he has not sought medical care unless absolutely necessary.

AL is a 57 year old with the chief complaint of high blood pressure of 170/92 taken in a community health fair, states was prodded by his wife to visit the clinic. Also reports acid stomach relieved with Omeprazole and achy knees relieved with Ibuprofen. Denies fatigue, fainting, headache, vision problems, nose bleeds, shortness of breath, chest pain, palpitation, and nausea or vomiting. Positive objective exam findings include BP 170/98 and prominent AV nicking of bilateral fundi. Pertinent negative physical exam findings include no retinal bleeding, no nasal discharge, no thyroid enlargement, no carotid bruit, no JVD, no audible heart murmurs, no swelling in all extremities, no labored respiration, no rales/rhonchi/wheezes, no abdominal distention/tenderness, no hepatomegaly, and non-palpable kidneys.

Anselmo Lopez is a 57 year old hispanic male who is overweight and a current smoker for evaluation for a blood pressure reading of 170/92 that was taken at a local health fair. He takes ibuprofen occasionally for his knee pain. He denies any headaches, somnolence, fatigue, chest pain, edema, or shortness of breath. Physical examination is notable for left arm BP 174/96and a finding of AV nicking. Family history of hypertension. Pt was told years ago he had high blood pressure but has not followed up since then.

57 year old male presenting with a complaint of elevated blood pressure that was discovered at a health fair and he was encouraged to see a provider. Patient denies any symptoms at this time. Patient denies PMH other than “acid stomach.” Patient does not have a PCP and does not see a doctor often. Patient has no concerns other than the high blood pressure.
