Week3 is another HEENT Focused Encounter (sore throat/pharyngitis/tonsillitis) commonly seen in primary care. Be sure to review the Week3 grading rubric as it is DIFFERENT from Week2. You have only ONE attempt for the Week3 case. The grading is more stringent.

AW is a 23-year-old female with complaints of a sore throat, fever, and headache x2 days. Patient denies cough, wheezing, shortness of breath, night sweats, rash, or ear pain. Patient presents with a fever of 101.5, swollen glands, tender cervical lymphadenopathy, pharyngeal erythema and tonsillar exudate. Patient’s respirations are unlabored, lung sounds clear, heart sounds normal, and inner ear appears normal.

AW is a 23- year old female, with a chief complaint of sore throat. Reports fever, chills, a mild headache, swollen neck glands, hoarseness, and related difficulty of swallowing. Also reports a sick roommate who had a sore throat early in the week. Denies fatigue, nasal congestion, cough, nausea or vomiting, diarrhea, and abdominal pain.

History of infectious mononucleosis at 15 years old. Influenza and COVID vaccines not up to date. Positive objective physical findings include temperature of 101.5 F; slightly enlarged tonsils with prominent exudates bilaterally; erythematous posterior pharynx; and bilateral tender cervical lymphadenopathy with nodes approximately 1.5 cm in size, rubbery, and mobile. Pertinent negative objective findings include absence of rash; lack of eye redness; bilateral nares are patent without drainage; non- tender frontal and maxillary sinuses; non- labored and easy respirations without signs of distress; and lung sounds are clear all throughout without crackles, rhonchi, or wheezes.

Physical exam shows fever 101.5, bilateral posterior cervical lymphadenopathy, erythematous pharynx with enlarged tonsils and prominent exudates bilaterally. Negative for abnormal breath sounds, and hepatosplenomegaly. Overall health unremarkable. No allergies to medications.

She states that her roommate has been sick as well this past week but is unsure of her diagnosis. Amanda has not received her flu vaccine this year. She reports no known allergies, and a history of mononucleosis at the age of 15. On exam, she has bilateral neck, tenderness and cervical lymphadenopathy. She denies the use of tobacco and smoking. She reports taking prescription oral contraceptives.
