EW is a 79-year-old female who presents with progressive confusion and memory loss over 1-2 years, worsening in the past year. She cannot manage finances, shows poor hygiene, decreased interest in activities, and has become irritable, suspicious of theft, and emotionally labile. Safety concerns include missed freeway exits, leaving the car and faucet on, and driving. Daughter reports decreased appetite and unintentional weight loss of 8 lb.
Daughter denies hallucinations, incontinence, or gait disturbance. On exam, she is alert but disoriented to time and place. MMSE 13/30, GDS 4/15, small-step gait, intact CN, and no focal neurological deficits.

Ella West is a 79-year-old widowed female with a PMH of HTN, CAD, insomnia, hearing impairment and hyperlipidemia, presenting with her adult daughter for evaluation. Per daughter, patient has had progressive memory loss, agitation and paranoia for the last year, and recently had a fall with head trauma (CT negative). On exam, patient is AAOX2, BP elevated, becomes agitated when asked assessment questions, denying need for visit. Patient’s history/symptoms mostly from daughter. She’s had an 8lb weight loss, appears unkept with excessive body odor. Daughter attributes both to decline in ability to perform ADLs. Patient scored 13/30 on MMSE and 4/15 on geriatric depression scale. Exam limited by patient’s comprehension.
