A.P a 50 y.o female presented to the ED with complaints of fatigue, lightheadedness, headaches, and SOB following exertion for the past few months. Reports a heavier menses for the past year with the first 2-3 days being the heaviest requiring her to change her tampons and pads every 2-4 hours. FMH of colon CA from which her father died at age 50 and chron’s on her mother’s side, with no past colonoscopy. On physical exam patient presents with pallor to the skin and conjuctiva, with bilateral oral angle fissure, and guiac stool faintly positive.

Patient is a 50-year-old female, no significant medical history. Endorses progressive fatigue, and lightheadedness with exertional dyspnea, positive for orthostatic hypotension. Patient is premenopausal with noted menorrhagia. LMP 4 weeks ago. Patient uses both pads and tampon first 2-3 days, changes the tampon every 2-4 hours.
Duration of menstruation is 7-8 days. Patient has strong GI family history in the setting of no colonoscopy. Vegan diet for two years. NSAID use. Skin is pallor. Erythematous fissures, bilateral angles of mouth. Thyroid not enlarged, no masses. Guaiac faintly positive.
