2 year old female child with sudden onset of rash this morning on abdomen and has spread to extremities. Mother reports fever of 103, cough, and irritability x 3 days ago. Fever resolved with OTC acetaminophen yesterday and developed rash this morning. Physical exam reveals well developed child with widespread maculopapular pink rash on abdomen and extremities. Mother denies itching or pain associated with rash. No history of this type of rash in past and no one in family with similar findings. No previous medical or family history. Attends daycare. UTD on vaccinations for age and has met milestones for 24 months.

Attempt 2

This is a 2-year-old child, brought who presents with a rash to the abdomen and bilateral extremities. Coughing, running nose, irritability, and fever were noted for three days, but the fever has now resolved as the current temp is 98.6. Mom reports that the patient isgoes to daycare. up to date on vaccines but missing the flu shot for this year. A physical exam revealed a rash to the abdominal region and extremities. There was a possible upper respiratory infection based on the presenting cough and running nose.
