The purpose of this discussion is to develop an evaluation plan for the grant proposal. Instructions Review the lesson and readings from this week. Continuing with the practice problem you identified in the Week 3 Assignment, respond to the following:

1. Identify the primary outcome(s) for your evidence-based intervention.
2. Describe how this will be measured (i.e., how the evidence-based intervention will be evaluated when it is fully implemented).
