KF is a 45 y/o female who presents with complaint of intermittent abdominal pain. Pain is worse after consuming rich foods, is sometimes relieved with defecation, and does not change with consuming dairy or wheat products. Patient denies fever, weight or appetite changes. Patient admits to cyclical constipation and diarrhea, alternating consistency between “hard pellets” and “watery with mucous.” Admits feelings of incomplete bowel emptying, bloating and belching, urgency with diarrhea. Denies nausea or vomiting, heartburn, food or acid taste in mouth. Denies tarry or bloody stools. Exam is positive for mild tenderness with deep palpation of the lower abdomen on both sides. Rectal exam and unremarkable. PE is unremarkable for additional findings.

The pain is all over, primarily in the lower abdomen, gets worse with rich foods and improves with bowel movements. She has also goes between periods of constipation and diarrhea. Denies any painful or bloody stools. Medical history includes headaches, hot flashes, c-section, and tibia fracture requiring surgery. Physical exam shows mild tenderness of bilateral lower abdomen to deep palpation. Denies any radiating pain, chest pressure or tightness, or difficulty breathing.
