DA is a 54 yo female who presents to ED for evaluation of acute severe periumbilical abdominal pain for 2 days. Associated symptoms include abdominal bloating, nausea, vomiting, constipation, and inability to pass flatus.
Reports vomitus as bilious. Physical exam reveals mild tachycardia with HR 105, abdominal distention, periumbilical tenderness, absent bowel sounds, and mild tympany throughout abdomen. No visible or palpable masses, no rebound tenderness, no shifting dullness or fluid wave. Reports abdominal surgery within last year.

D.A. is a 54-year-old female with abdominal pain for 2 days. She has a history of multiple abdominal surgeries. She reports high cholesterol and on atorvastatin for it. She also reports alcohol use and denies fever, flatus and bowel movements for the last two days. On physical exam, it shows tachycardia, periumbilical discomfort on palpation, and abdominal distention.
