FR is a 38-year-old male who presents with shortness of breath, fever, night sweats, dry, nonproductive cough and weight loss of 5lbs. Patient notes his shortness of breath is worse with exertion. Patient used albuterol inhaler from friend with no relief. Denies sore throat, congestion, dizziness, nausea, vomiting, diarrhea. History of HIV 8 years ago, was on HAART therapy but quit after 3 years. Pt has fever of 101.2, tachycardia and tachypnea with accessory muscle use. Pt has palpable and tender anterior and posterior cervical lymph nodes, crackles in lungs bilaterally and SPO2 89%. Patient has dry mucus membranes. Pt skin without rashes or lesions, no petichiae or unexplained bleeding or bruising noted. Pt smokes 1/2 pack cigarettes per day for 20 years. Denies illicit and recreational drug use.

A 39-year-old male presents with several weeks of progressively worsening shortness of breath that initially occurred with exertion and is now present at rest, accompanied by a persistent nonproductive cough. He reports fatigue for approximately six months, a current fever of 101.2°F, drenching night sweats, and an unintentional five-pound weight loss. His medical history is significant for untreated HIV infection and tobacco use. On examination, he appears mildly uncomfortable with conversational dyspnea and use of accessory muscles. Vital signs demonstrate active tachycardia, tachypnea with labored respirations, and fever, while blood pressure remains stable. Physical examination is notable for firm, mildly tender anterior and posterior cervical lymphadenopathy, with no peripheral edema or focal neurologic deficits.
