SUBJECTIVE:
CC: Irregular mole on right shoulder, onset 6 months ago, changes in size, color and irregularity over the last 3-4 months.
HPI: Patient is a 72-year-old male who presents with a mole on the right shoulder with irregular borders, multiple colors, with significant changes over the last 2-4 months. Pt reports lesion is painless, non-tender, and is annoying him as it catches on his clothing. Pt denies any aggravating or relieving factors and has sought no previous treatment.
ROS: General: Denies fever, weight loss/gain, loss of appetite, fatigue, weakness, or pain. Skin, hair, nails: Mole on right shoulder that is asymmetric, multi-colored, irregular border, and 4mm in diameter. Pt denies any bleeding, bruising, lacerations, tenderness or pain.
