Description
EC is a 13-year-old male who presents with his mother to be evaluated due to refusing to attend school. She reports dropping grades, disruptive, easily distracted, unkept belongings, procrastinates/misplaces his homework, fidgety, irritable, frustrated, anxious, trouble following directions with more than one task. Negative for headaches, weakness, dizziness, palpitations. Objective findings: Positive fair eye contact, irritable affect. Occasional distractibility, or looking away from interviewer. Some errors on cognitive testing; serial 7’s was 3/5 and WORLD backward was, DRLOW, later recall intact. Cranial nerves II-XII intact. Fidgeting through the exam. Deep tendon reflexes intact 2+. Negative for involuntary movements, facial abnormalities, abnormal thyroid, tachycardia, hyperreflexia, difficulty with vision or hearing. Family history of maternal smoking during first trimester and concentration issues. Paternal history of cocaine abuse.
